Syd is correct in his earlier post, JD only had one F-612, serial number OD 101, guitar has been on displaying various locations in Denver where you can get within 2' of it and clearly see the label and the details (odd fact that according to the Guild serial number chart on their website, serial number OD 101 belongs to an A350 model from 1969).

Guitar was redone by Arturo Valdez at his shop across from the Hollywood Guitar Center on Sunset Blvd. Work included new top and fingerboard, new checkerboard binding on top, peg head and side of fingerboard (for some reason the decision was made to place the checkerboard on the outside layer of binding). Pearl inlays were installed on the fingerboard and bridge, saddle was compensated in three sections. Unfortunately the Valdez shop has closed but you can still find stories about the shop and Arturo online. I still have the shop brochure from when I was there years ago that clearly describes the work done to John's F-612.

I also believe that the guitar from the early records and videos is an F-412 Special with a flat back.

VEER:

I have a 12-string made by Arturo Valdez. He had a shop on Sunset Blvd, did work for everyone. He made but a handful of 12-strings -- only eight are known.

He was an official Guild repairperson and did a lot of work for John Denver. (It's curious that when Valdez designed his own 12-strings, he built them to same dimensions as the JD's F612 -- though his resembles the F612 not at all -- weirdest cutaway ever.)

Valdez is mostly known for his classical and flamenco guitars (though his most famous creation is -- wait for it! -- Gene Simmons' bloody battle-axe bass!) and while there are numerous pics online of the nylon string guitars (and the famous bass) the only pic I've been able to find of any of his 12-strings is one blurry black & white shot of Byrd Roger McGuinn with his AV 12-string.

See: https://www.amazon.com/DAngelico-Master-Guitar-Builder-Whats/dp/1574242172

The book is mainly about D'Angelico and D'Aquisito, but there's also a section on Valdez, with a rundown of some of his 12-strings. (Al Stewart's is #6; Lalla Hansoon's [?] is #7; Roger McGuinn's is #8. I believe mine is #3. Who the others were made for or what's become of them is not known.)

Here's a picture of the headstock of one of his flamenco guitars -- my 12-string headstock is slightly more elaborate, but basically the same.
