Well, I haven't had any pop off issue from the bridge side, but I do have a pop off issue (with the carved S-100 recently purchased) where the E string slides off the lower edge of the finger board and gets stuck under a fret!! I think the problem comes from the bindings that were replaced (probably after e refretting job and that don't really fit in place, so I guess I'll take that one to my local guy (since Tom - aka "fixit" is so far away :( ) and have him look at it.
Might also be that the nut slot for the E string is too close to the edge.

I had a Gibson 335 with a similar issue - it didn't get stuck under a fret, but did pop off the fretboard from time to time - not very musical. I noticed that it ran much closer to the edge of the fretboard than on my other guitars. I never thought of getting the nut re-cut, but that is a logical fix.
I am thinking of installing a compensated nut on the S-100 when I finally get one, and maybe also on my Strat. Just to be really in tune at the lower frets. I notice that Ernie Ball have stopped Earvana from making them due to patent infringement, but there is a UK eBay seller who hand-makes bone compensated nuts to order.
I've always wanted an S-100, but it was your youtube video that turned it from a latent desire into a full-on craving....
